During your initial appointment, we will discuss your diagnosis and treatment options and answer any questions you may have.
To ensure a smooth consultation, please bring the following information with you for your first visit:
Photo ID, dental and medical insurance cards
A list of current medications, including dosage and reasons for use
A list of all known allergies or current medical conditions
A parent/guardian if the patient is younger than 18 years old
Current x-rays, if applicable
A referral slip, if applicable
